This scholarship is to assist low-income families with the costs of registering or purchasing equipment for participation in youth baseball programs. The scholarship will pay for ½ of the cost of registration or provide up to $50 towards the purchase of equipment.
IMPORTANT INFORMATION:
- Applicants must be residents of Montezuma County and provide a physical address.
- Scholarships are available on a first come, first serve basis for qualified applicants. You will receive a letter of approval, denial or request for more information.
- If you are awarded a scholarship, Onward! A Legacy Foundation will issue payment for 1/2 player registration fees on the player’s behalf. If equipment is requested Onward! A Legacy Foundation will contact you to coordinate.
- Eligibility is determined based on the following gross income guidelines:

| 5 | $6,276 |
| 6 | $7,192 |
| 7 | $8,110 |
| 8 or more | $9,026 |
Valid forms of proof of income include:
- Copies of 2 recent pay stubs
- Copy of notice stating amount:
- SSI, Social Security, VA, Disability (SSDI)
- Pension, Unemployment benefits, Worker’s Compensation
- Proof of Child Support Payment, Welfare benefits
- AFDC or any other income
- Copy of a bank statement showing direct deposit of such payments
- Copy of court order or other proof of child support amount
